Based on observation at approximately 10:20 a.m., S2 failed to ensure children under the age of two years-old did not participate in electronic device activities. The Specialists observed S2, allowing C11, 1-years-old, watch an animated program on the center's iPad. This was corrected prior to the Specialists' departure. Corrective Action: Effective 04/23/2026, S3 stated she would retrain staff of the regulations for the electronic policy and perform random checks to ensure compliance with this regulation.

Based on record review at approximately 11:05 a.m., S1 failed to have documentation that she had conducted tornado drills for May and June of 2025. This could not be corrected prior to the Specialists' departure. Corrective Action: Effective 04/23/2026, S3 stated she will perform tornado and fire drills monthly to ensure compliance with this regulation.
