Staff member D.R. used inappropriate discipline when she nudged/shoved a 1 year old child back into his seat causing him to hit his face/chin on the table. This resulted in a red mark on his chin and biting the inside of his lip. Staff member was terminated immediately.

1 bottle in top shelf of refrigator in infant room on left hand side was not dated or labled with milk in the bottle.

Toddler restroom had broom, dust pan and paint projects drying. Twos restroom had dust pan and broom. three-four room had plunger and a package of diapers was in plastic but the top was open and a diaper was pulled almost out of it. All items must be sanitizable in a restroom.
