Daycare 2(110) - Unit is licensed for eight children 12 months to 2.5 years and with a ratio of 1 staff to 5 children. One staff is present with six children in care at time of survey.

Daycare 2(110) - Toddler is napping without supervision in a room separate from the teacher. Activities(107) - Staff member is observed leaving three children ages 2.5 years to 5 years unsupervised multiple times during nap. Once to attend their own child in another unit, again to prepare snack in the kitchen, and twice to deliver snack to another unit.

Three of nine staff files reviewed do not have health and safety training within 30 days of hire. Staff were hired in August, October, and November of 2023.

Surveyors observed a 23-month-old in Activities(107). Surveyors observed a staff member attempting to take a 9-month-old from Infant(102) into Activities(107). Activities(107) is licensed for children 2.5 years to school age.

An outlet is uncovered in an unlicensed but accessible room between Infant(102) and Daycare 2(110). Children were present in the room.

Daycare 2(110) - Medication, sunscreen, personal care products, hydrogen peroxide are on the counter and in unlocked cabinets. Infant(102) - Baby lotion is on the counter. Hallway outside of classrooms - Lotion and Expo cleaner are on open shelves. Adult scissors are on the counter of an unlicensed but accessible room between Infant(102) and Daycare 2(110). All items are labeled "keep out of reach of children" and are in unlocked storage. Children were present in all areas cited.

Facility is using an unlicensed unit between Infant(102) and Daycare2(110) to nap children while awaiting amendment approval from KDHE. Daycare 2(110) - Per program director's statement, children eat lunch in the foyer, outside of their unit. Infant(102) - Upon surveyors' arrival two infants are being carried in the hallway and kitchen, which are unlicensed, rather than remaining in their unit.

Large playground - The steps of a small plastic slide have a crack in an area where children's fingers could be pinched.

Activities(107) - Toothbrushes in two bathrooms are laying on the counter and are not stored in a sanitary manner out of children's reach.

Infants(102) - The mini fridge/freezer does not have thermometers. Kitchen - two refrigerators and three freezers do not have thermometers.

Daycare 2(110) - Diapering procedures are not posted.
