The Specialist observed S5 alone supervising 13, three to four-year-olds, and S6 alone supervising 8, one to two-year-olds. A CPR training has been scheduled for 7/08/2025 with an approved trainer. 1711.A. B. D.: Based on observation/interviews at 9:30 a.m., S2 failed to meet the required child to staff ratio for children of the following ages: 9, one-year-olds, were being supervised by one staff member, S3. The required ratio for one-year-old is 7:1. Child to staff ratio was met prior to the Specialist's departure. Corrective Action: Effective 6/02/2025, S2 stated they will ensure child to staff ratios are met at all times by having an additional staff member at the center as needed, to ensure compliance with this regulation.
