At the time of the inspection, in the infant classroom, an infant was observed sleeping with a blue baby blanket.

At the time of the visit, observed one teacher with 10 toddlers in the Crayola One classroom. This is a direct violation of stipulation (C). Probationary stipulation C states " Licensee shall comply with required staff-to-child ratios at all times. Ratios and room capacities must be posted in each classroom."

At the time of the visit, on the toddler playground, observed a small red tricycle with a broken seat and on the back of the tricycle was a broken bucket. Another small red tricycle also had a broken bucket on the back of the tricycle on the toddler playground.

At the time of the visit, observed a broken blue sit and spin style toy on the toddler playground. The toy had a large crack and a large missing piece on the part of the toy where a child would sit.

Reviewed 4 staff files and observed that the facility did not maintain the CBI and FBI clearance letters for Elyzabeth M. Elyzabeth M's CBI was cleared on 4/9/2021 and FBI was cleared on 4/22/2021.

On the Preschool Playground, observed a garden bed with a broken leg post being propped up by a milk crate posing a hazard. This is a violation of stipulation S on the probationary license probationary stipulation S states “Licensee shall ensure all indoor and outdoor play areas are maintained in a safe condition and that hazardous, high-risk areas are made inaccessible to children.”

Identified through staff statements that staff are not following the facilities policies and procedures regarding cell phone storage.

In the Crayola 2 classroom, observed that 1 child was not signed out for a transition into the Bumble Bee classroom. Staff stated that there were 6 children in the classroom and there were only 5 children in the classroom. This is a violation of stipulation F and G on the probationary license. Probationary stipulation F states "At all times, staff members shall know the attendance of the classroom to which they are assigned. Facility Director shall be able to account for all children in care at the facility." Probationary stipulation G states "Licensee will use a name-to-face attendance verification tool (paper or electronic form) and complete attendance verification for all transitions and at frequent intervals throughout the day. Licensee shall provide training for all staff on how to use the attendance verification tool and must maintain verification of the training in each employee’s staff file."

Reviewed 19 staff files and observed that the TRAILS background check for Gerelene C., and Lilian R. were not submitted within 10 calendar days of the first day of employment. This is a violation of stipulation P on the probationary license probationary stipulation P states “Licensee shall timely submit a fingerprint based background check and Trails abuse/neglect background inquiry requests for any new hire. Documentation of the background check must be maintained in the staff file at all times.”

In the Bumblebee's classroom, observed that 1 child was not signed in when transitioned from the Crayola 1 classroom to the Bumblee classroom. This is a direct violation of stipulation G on the probationary license. Probationary stipulation G states “LIcensee will use a name-to-face attendance verification tool (paper or electronic form) and complete attendance verification for all transitions and at frequest intervals throughout the day. Licensee shall provide training for all staff on how to use attendance verification tool and must maintain verification of the training in each emmployee's staff file."

In the Crayola 1 classroom, 2 children were not signed out on the attendance tracker.

In Crayola 1 classroom, observed Oxivir cleaning product labeled "keep out of reach" under an unlocked cabinet. In the Baby Shark classroom, observed hand wipes labeled "keep out of reach" in an unlocked drawer.

In the Crayola 1 classroom, identified that 1 child was not signed in from the morning and 1 child transitioned and there as not an accurate written attendance verification for the transition.

In the Crayola 2 classroom, observed 1 teacher out of ratio with 10 toddlers. In the Sesame Street classroom, observed 1 teacher leave the room out of ratio with only 1 teacher and 10 infants.

At the time of the visit, reviewed 24 staff files and observed that Gloria A., Denia S., Marianita B., Abigail Q., Leisha H., Olivia R., Sherry S., and Kylisha D. were missing documentation of a current medical statement signed and dated by a physician.

In the School Age room, observed Oxivir and bleach solution cleaning bottles labeled "keep out of reach of children" in a cabinet accessible to children. In the Parakeet room, observed a child's spray sunscreen labeled "keep out of reach of children" in a child's cubby.

Confirmed through staff statement that SM1 grabbed CH1 by the arm, slammed CH1 against the wall, and hit CH1 against the lip as a punishment.
