PreK room: There is a section on the wall missing baseboards. There are two toilets that have waste in them and there is an odor in the bathroom; not maintained clean. Infant room: There is a section of overhead lights that is not functioning; not in good repair.

Infant room: A staff member changed an infant's diaper and then placed the infant on a boppy pillow without washing the child's hands. PreK room: None of the children have had their hands washed after eating lunch.

Preschool playground: There are two large wooden spools that have sharp screws exposed; not in good repair.

PreK room: Upon entry into the room, children were observed to be having free play. Surveyor asked for the program plan, the staff member stated that it was "Free-Play Friday", which was not noted on the posted program plan. The program plan outlined activities for the week, and no activities were being offered by the staff member. The program plans were not being followed. The two staff members present in the room were not interacting with the children; they were observed to be sitting at tables alone.
