Room Daycare 2 does not meet the minimum staff-child ratio when there are six children, ages 1 year 4 months to 2 years 4 months, with one staff from 9:00am - 9:28am.

Children in room Daycare 2 did not wash their hands after being diapered.

The diapering surface in room Daycare 2 was not disinfected after each use when a commercial multipurpose degreaser and spot remover was used and wiped right away.

The diapering procedures in room Daycare two were not followed on one occasion when staff did not place a disposable towel underneath the child (step 2), staff did not clean the child's bottom (step 5), & did not remove gloves before diapering & dressing the child (step 7). On a second occasion, staff in room Daycare 2 did not remove the disposable towel before diapering and dressing the child (step 6).

Staff in room Daycare 2 did not wash their hands after diapering.

The terms of license was not followed in room Daycare 2, licensed for toddlers, when an infant, 8 months old and not walking, was present in the room upon licensing specialists arrival. Corrected during visit.

Staff in Daycare 2 (Aquarium) did not follow commerical disinfectant directions when the disinfectant spray used states to sit for 10 minutes after spraying, and the staff wiped it off immediately.

Facility made a change in the use of space that affects the terms of the license when the facility changed from catering meals to cooking on-site. Facility only has a microwave on-site to prepare meals for license capacity of 46 children.

Per staff statements, children do not brush their teeth at least once daily.

Nap mats in Daycare 2 (Aquarium) and Daycare 3 (Number) are not seperated by at least 24 inches in all directions.

Milk was not offered during the noon meal in Daycare 2 (Aquarium), Daycare 3 (Number) and in Daycare 4 (Galaxy).

A sufficient quantity of food was not prepared for the noon meal when preschool aged children were served two mini raviolis, three mandarin orange slices, one cracker, & five green beans. Toddler aged children were served one mini ravioli, two mandarin orange slices, one cracker, & three green beans. Seconds were not available or offered.

Children were not encouraged to self serve in Daycare 2 (Aquarium), Daycare 3 (Number) and in Daycare 4 (Galaxy).

Tables were not cleaned before the noon meal in Daycare 2 (Aquarium) and in Daycare 4 (Galaxy).
