C.: Based on record review at 9:30 a.m., S4 failed to have documentation that 1 of 8 staff, (S3), completed orientation requirements within seven and thirty days of the first day present at the center and prior to having sole responsibility for any children. S3 (FDW: 1/20/2026) failed to complete LDE Key Training Modules 2 and 3 and the DCFS Mandated Reporter Training. Corrective Action: Effective 4/28/2026, S1 will make hard copies of all new hire training for substitutes and keep at the center to ensure compliance with this regulation.
