1903.C. Based on observations at 10:50 a.m., S1 failed to ensure the outdoor area was free of hazards. The Specialist observed broken plastic at the bottom of the slide on the wooden jungle gym. There was also two screws sticking out of the rotten wood on the right side of the slide. This was not corrected prior to the Specialist departure. Corrective Action: Effective 6/9/2025, S1 stated she will block off the slide until it is fixed to ensure compliance with this regulation.
