      Girls bathroom sink in the gym has a seal on the sink that is coming apart.

      A child care center shall, at no expense to the state, maintain and make availab 3 of 10 staff files reviewed missing documenation of drug test results. Send documentation to consultant.

    • Child/staff Ratios and Supervision

      The 1st grade group did not have the proper number of qualified caregivers directly caring for the number of children in the classroom. They had 0 qualified caregivers with 12 children school aged and the required ratio for that age group is 1:20(variance on file). The 3rd grade group did not have
