Based on records reviewed at 11:00 a.m., S1 failed to have documentation that S1 (DOH 12/01/1995), S2 (02/16/1996), S3 (DOH 08/27/2024), S4 (06/14/2021), S5 (DOH 03/01/2017), S6 (01/26/2024), S7 (07/24/2014) completed the online child abuse and neglect Mandated Reporter Training provided by DCFS. Corrective Action: Effective 11/25/2025, S1 stated she will ensure all staff completes mandatory reporting training yearly to ensure compliance with this regulation.
