Multiple times, involving 10 toddler and preschool age children, Licensing Specialist observed children unsupervised and alone in bathrooms, kitchen, and hallways. On 3 occasions staff were unaware of children's locations. Children had access to building exits unsupervised and with unit doors closed and locked, Licensing Specialist opened doors for children, granting them access back to their units. Per collateral contact and observation of Licensing Specialist, children were not assisted with potty training. Multiple children were observed changing their own pull-ups with no staff assisting or assuring that children were clean prior to new pull-up being put on child.

Multiple staff not demonstrating an understanding of children or protected the health and safety of children. Per Licensing Specialist observation and collateral contacts staff do not engage with children, do not protect children from harm in chaotic gym environment, and do not comfort or respond to crying injured children. Staff observed sitting on floor or socializing with each other, ignoring children and not responding to dangerous behaviors or crying children. Collateral contacts report multiple staff are distracted by phone use and ignore children while playing games on their phones. Staff were unable to accurately tell Licensing Specialist how many children were in care in their unit. Eight staff indicated the gym is not safe for young children when preschool children are present, yet all staff continue to use the space in this manner, placing children at risk daily.
