The wall in Room A 18 was not free of cracks when there is a crack at least 5 feet long on the south wall of the classroom.

Current written program plans were not available in Rooms A 16 and C 104.

Per staff, an infant (14 months old), not yet walking, is sleeping on the cot and not in a crib or playpen.

While lunch was being prepared, the kitchen was used as a passageway when a staff member brought an infant to the office.

The diaper changing table in Room A 16 was not disinfected when the staff did not follow the instructions to allow the disinfectant spray to sit for one minute before wiping down per manufacturer's instructions.

Staff in one unit did not follow the diaper procedure when not washing her hands or the infant's hands after diapering.

The terms of the license was not maintained when children were in a room not on the license for a special program.

Several outlets in the building housing Rooms C 103, C 104 and C 106 were uncovered and accessible to children.

Premises was not maintained in good condition, clean at all times or free from accumulated dirt and trash or vermin or rodent infestation. The basement where children go for tornado drills was dirty with cobwebs, dirt, leaves and a pile of debris in the corner. There were cracked and splintered boards with splintered edges and pointed edges, bent electrical wire housing leading to electrical outlet, water damaged doors, moldy boxes and exposed nails. Upstairs there was evidence of rodent feces.

Current lesson plans were not available in Room A13 (Aunt Bev's room) or C 106.

Child sleeping in Room C 103 did not have a cot or nap pad when sleeping on the floor.

Children in Room C104 and C103 lined up to go to the bathroom for lunch at 11:00, children waited in the hallway, with no activities, until 11:20 and lunch was not served until 11:35.

Bottles in Room A 16 did not have the contents of the bottle on the label.

The kitchen was accessible to a child and used as a passageway when the child walked into the kitchen and out the other door to go to the classroom.

Out of thirteen children's files reviewed, one child does not have documentation of current immunizations on file.

Two units, A 13 and A 18, do not have a staff member that meets qualifications for unit they are assigned when they do not have the three months experience with infants or toddlers.

The facility is not maintained at all times when there are gnats flying throughout the lobby and kitchen where food is prepared.

Food from breakfast was still on the floor, not swept, at 10:30 am in Room C103 and breakfast was served at 8:30 am.
