Corrective Action: Effective 04/28/2025, S1 stated she will check staff attendance daily and have S2 ensure she is signing in and out to ensure staff are signing in and out to ensure compliance with this regulation.

Based on record review on 04/28/2025, at 10:50 a.m., S1 failed to maintain documentation of a daily attendance record for visitors to include arrival, departure times and purpose of the visit. O4 visited the center on 04/23/2025, but there was no documentation on the daily attendance record for visitors. 1901.P Based on observations on 04/28/2025, at 9:32 a.m., S2 failed to ensure the personal belongings were inaccessible to children. Specialists observed S2's cellphone and beverage cup sitting on top of the table accessible to 6 children, ages 3 to 4-years-old. Corrective Action: Effective 04/28/2025 S1 stated that she will remind staff to put their personal belongings up and away from children to ensure compliance with this regulation.

1901.Q. Based on observations on 04/28/2025, at 9:34 a.m., S1 failed to have a current copy of The Safety Box newsletter issued by the Louisiana Office of the Authority General posted. Corrective Action: Effective 04/28/2025 S1 stated she will ensure to keep a current safety box newsletters posted to ensure compliance with this regulation.

1911.G. Based on observations on 04/28/2025, at 9:36 a.m., S5 failed to ensure pacifiers were not attached to the clothing of an infant. C2, age 11 months, was observed with a pacifier clipped to her clothing. S5 removed the clip during the licensing visit. Corrective Action: Effective 04/28/2025 S1 stated she will remind staff to not clip pacifiers to infants clothing to ensure compliance with this regulation.

1919.B.2 Based on observations on 04/28/2025, at 11:14 a.m., S1 failed to have menu substitutions or additions posted, written or electronically, on or near the menu as the Specialists observed the children eating white beans, rice, peaches, bread, and milk. The menu posted stated the lunch for the day would be meatballs with cheese, spaghetti, green beans, peaches, and milk. Corrective Action: Effective 04/28/2025, S1 stated she will make sure the food menu is posted at all times with substitutions to ensure compliance with this regulation.
