      During the inspection , the LS observed that a form needed Physician's information. The child's parent came in and completed the form correcting this non-compliance on-site. The LS reminded the Provider to make sure and have parents complete all items on the Emergency form.

      The LS observed that the cold water pipe was over a child's bed. The provider stated that the fire inspector approved it as long as a covering was placed over it. The LS took pictures and will review with her supervisor.
