Based on record review at 1:24 p.m. S1 failed to have documentation that 1 of 6 staff, S6, completed the online child abuse and neglect Mandated Reporter Training provided by DCFS for 2025. Corrective Action: Effective 3/12/2026, S1 will have all staff to complete DCFS Mandated Reporter Training annually to ensure compliance with this regulation.
