Based on record review and interviews at 10:30 a.m., S1 failed to immediately notify the O1, Parent of C1, of the following critical incident: On 2/12/2026, at 8:55 a.m., C1, three-years-old, was left unsupervised in an office for one minute and twenty-four seconds. O1 was not notified until 11:48 a.m. Corrective Action: Effective 2/24/2026, S1 stated the conducted a staff meeting on 2/24/2026, on Critical Incident Reporting to ensure compliance with this regulation.

Based on record review and interviews at 10:30 a.m., S18, S19, and S20 failed to ensure children were supervised at all times. On 2/12/2026, at 8:55 a.m., C1, three-years-old, was unsupervised in an office for approximately one minute and twenty-four seconds. S18, S19, and S20 were on duty that morning, as the children were getting off the bus and entering the building. S11’s office door was left open and C1 walked into the office by himself at 8:55:56 a.m. S11 walked into her office at 8:57:20 and found C1. S11, S18, S19, and S20 each received disciplinary actions. On 05/21/2025, at 11:35 a.m., C1, three-year-old, walked to the doorway and dropped his lunch plate on the floor. S21 walked by him and pushed his head with the back of her hand roughly and he moved out of view. S17 stated she saw the incident and did not report it. Based on record review/interviews on 08/28/2025, at 11 a.m., although the center has a behavior management policy, S21 (DOH 12/14/2024) used a prohibited method of discipline. Based on video footage review, on 05/21/2025, at 11:35 a.m., C1, three-year-old, walked to the doorway and dropped his lunch plate on the floor. Based on record review/interview on 08/28/2025, at 12:30 p.m., S1 failed to report an incident to the parent no later than when the child is released to the parent or authorized representative on the day of the occurrence. On 05/07/2025, there was an allegation that S17 and S23 attempted to scare C1, three-year-old, with a worm. S24 notified S1 of the incident on 05/22/2025, and the parent, O1, was notified on 05/29/2025. 1713. A. B. C. Based on record review/interview, at 11:00 a.m. on 3/17/2025, S17 failed to supervise her classroom of nine children, ages three-to-four-years-old. On 3/13/2025, at 10:28 a.m., S17 exited the classroom after eight children and locked the door to get on a bus for a field trip. Once the class was loaded on the bus, S27 noticed C1 (3-years-old) was not on the bus. At 10:34 a.m., S27 and S9 exited the bus and retrieved C1 from the classroom. C1 was brought to the bus to join his class. C1 was left in the classroom, unsupervised, for 6 minutes.

1915.A. Health Services - Observation: Based on record review at 10:00 a.m., S3, S10, and S12 failed to provide documentation of observations being documented when something is observed, noted on children upon arrival to the center. Results including an explanation from parent and/or child were not documented. The Specialist observed that daily observations had not been completed by S3, S10, and S12 since 3-28-2025.
