O1 was not notified until 11:48 a.m. Corrective Action: Effective 2/24/2026, S1 stated the conducted a staff meeting on 2/24/2026, on Critical Incident Reporting to ensure compliance with this regulation.

On 3/13/2025, at 10:28 a.m., S17 exited the classroom after eight children and locked the door to get on a bus for a field trip. Once the class was loaded on the bus, S27 noticed C1 (3-years-old) was not on the bus. At 10:34 a.m., S27 and S9 exited the bus and retrieved C1 from the classroom. C1 was brought to the bus to join his class. C1 was left in the classroom, unsupervised, for 6 minutes.
