Proper diaper procedures were not followed in two units. In the Teal unit, staff did not wipe her hands with a moistened towelette or paper towel (Step 7) before placing the clean diaper on the child. In the Purple unit, staff did not wash the child's hands (Step 9), disinfect the changing table (Step 11) or wash staff hands (Step 12) before placing the next child on the changing table.

In the Purple and Lavendar units, children are put in a confined space for discipline when placed in an opened top cube.

Changing pads have damaged surface in the teal and green units. This was corrected at the time of the survey by the program director.

Cleaning supply products, with warning labels, are not kept in locked storage or out of the reach of children.

The fire exit in the Purple unit was blocked with stacked chairs. This was corrected by staff when chairs were moved at the time of the survey.

Children were not encouraged to serve themselves in any of the rooms when the plates are prepared for them by the kitchen staff. Staff did not sit with children and encourage socialization in five units.

Opened graham crackers (Teal unit), opened chips and pretzels (kitchen) are not stored in containers with tight fitting covers.

Toy was not washed and sanitized after being in a child's mouth when staff removed the toy and put it in the toy bucket.

Infants are not taken outdoors daily.

Infant bottles were not labeled with the contents or the date prepared.
