Based on record review on 2/10/2026, at 12:30 p.m., S1 failed to document the entire center and play yard was checked after the last child departed to ensure that no child was left unattended at the center on 2/6/2026 to 2/9/2026. This could not be corrected. Corrective Action: Effective 2/10/2026, S1 will to assign a staff to complete the End-of-Day checks daily ensure compliance with this regulation.

Based on observation and record review on 2/10/2026, at 11:45 a.m., S1 failed to ensure the staff and owner's daily attendance record accurately reflected persons on the center premises at any given time as S6 was present and not signed in. S1, S2 and S7 failed to signed out on 2/6/2026. Corrective Action: Effective 2/10/2026, S1 will retrain staff on the center's attendance requirements to ensure compliance with this regulation.
