1103.A.&C.&D.: Based on record review/interview(s) on 4/02/2026, at 10:15 a.m., S1 failed to notify the Department and DCFS within 24 hours of the following critical incident: On 3/30/2026, at approximately 10 a.m., a parent notified S1 that her child, C1 (2-year-old), alleged S2 hit her on her head while playing outside and S5 used his elbow to change her pamper. The Specialist was unable to validate the allegations. Corrective Action: Effective 4/02/2026, S1 stated she will re-train all staff on reporting requirements for critical incidents to ensure compliance with this regulation.

Based on record review at 10:53 a.m., S1 failed to ensure C1's (4-years-old) prescription medication, Valtoco, was in the original pharmacy container with the complete pharmacy label. Corrective Action: Effective 10/8/2025, S1 stated she will review that the medication is in the original pharmacy-labeled box before accepting it into the center, to ensure compliance with this regulation.
