591-1-1-.14(2)(b) requires that there must always be one staff person present in each classroom where children are present that has current and valid pediatric cardiopulmonary resuscitation (CPR) and pediatric first aid training. It was determined based on observation that the infant teacher was alone in the infant room with six children present and her CPR and First Aid training recently expired.

591-1-1-.10(9) requires Center Staff to not use the area used for diapering for food preparation and to keep the diapering area clear of formulas, food, food utensils and food preparation items. 591-1-1-.30(1)(a)3 requires that each crib shall have only an individual, tight-fitting sheet which is changed daily or more often as needed and prior to a change of occupant. It was determined based on staff interview and observation of soiled crib sheets that infant crib sheets are only washed once a week instead of daily as required.

591-1-1-.07(5) requires Center Staff to not permit children to wear around their necks or attach to their clothing pacifier or other hazardous items. It was determined based on observation that a infant was observed laying in the crib with a pacifier clip attached on the clothing and a bottle of milk in his mouth.
