Based on record review at 9:30 a.m., the daily attendance record for children failed to be maintained and include the departure time on 10/25/2024 and 10/28/2024 and the name of the person or entity to whom the children were released to for the month of October 2024. This was not corrected during the inspection.

Based on observation/record review at 10:24 a.m.,the center's staff and owner's daily attendance record failed to accurately reflect persons on the child care premises at any given time as S4 was not present and was signed in on the record at 7:15 a.m. S4 corrected when she returned to the center at 10:55 a.m.

Based on record review at 1 p.m., S1 failed to have a policy requiring all center staff and owners (mandated reporters) to report any allegation or suspicion of abuse or neglect of a child to the Louisiana Child Protection Statewide Hotline 1-855-4LA-KIDS (1-855-452-5437). This was not corrected during the inspection.

Based on record review at 1 p.m., S1 failed to have a Programs, Movies and Video Games Policy. This was not corrected during the inspection.

Based on record review at 1 p.m., S1 failed to have a policy/procedure to advise Parents of the licensing authority of the Licensing Division along with the current telephone number and email address. Parents shall also be advised that they may call or write the Department should they have significant, unresolved licensing complaints. This was not corrected during the inspection.

Based on record review at 1 p.m., S1 failed to have a written Electronic Devices Policy that provides that all activities involving electronic devices, including but not limited to television, movies, games, videos, computers and handheld electronic devices, are prohibited for children under are two and time allowed for electronic device activities for children ages two and above shall not exceed 2 hours per day, with the exception that television, DVD, or video viewing shall be limited to no more than one hour per day. This was not corrected during the inspection.

Based on record review at at 12 p.m., S1 failed to have a Child's Information Form for 1 of 6, C2, infant, children's records reviewed. This was not corrected during the inspection. 5 of 6, C1-C5, Child Information Forms failed to include the child's date of admission.

Based on record review at 12 p.m., S1 failed to have a signed and dated parental authorization to secure emergency medical treatment for 1 of 6, C2, Infant, children's records reviewed. This was not corrected during the inspection.

at 12 p.m., S1 failed to have an application/staff information form to include name, date of birth, home address and phone number, training, work experience, educational background, hire date, and first day onsite working with children for S5. This was not corrected during the inspection.

Based on observation at 10:32 a.m., S1 failed to ensure the center equipment used by children was maintained in a clean and safe condition and in good repair. Specialist observed several areas of the jungle gym/slide play yard equipment have broken plastic pieces with sharp pointed edges. This was not corrected during the inspection.

Based on 10:25 a.m., S1 failed to remove a bib to be worn by a C2, 5-months-old, while asleep. This was corrected during the inspection.

Based on record review at 10:20 a.m., S1 failed to have a daily written or electronic report for 2 of 2 infants, C1 and C2. This was not corrected during the inspection.

Based on record review at 12 p.m., S1 failed to have an Emergency Preparedness and Evacuation Plan to include specific procedures for handling infants through two-year-olds, including food and formula. This was not corrected during the inspection.

Based on observations at 12 p.m., S1 failed to have a completed evacuation pack that included a list of emergency contact information and emergency medical authorization for all enrolled children, an emergency pick up form, a battery powered or crank radio, and working batteries and flashlight. The bottle water expired in April 2018. This was not corrected during the inspection.
