    • Child Neglect and Abuse Mandatory Reporter Training1727.A.&B

      Based on record review at 11 a.m., S1 failed to have current documentation that 4 of 9 staff completed the online child abuse and neglect Mandated Reporter Training provided by DCFS. S2, S4, S7, and S9's training were expired. Corrective Action: Effective 6/23/2025, S1 stated she will ensure all staff complete this training by 6/23/2025, and going forward, she will set a reminder 1 week prior to the trainings expiration date to ensure staff complete the training timely to ensure compliance with this regulation.

      Based on observations at 9:30 a.m., S1 failed to document that the entire center and play yard is checked after the last child departs to ensure that no child is left unattended at the center. The End-of-Day checks had not been completed since 5/29/2025. Corrective Action: Effective 6/23/2025, S1 stated she will complete the end-of-day checks daily or ensure staff are completing the end-of-day checks if she's not present, to ensure compliance with this regulation.

      Based on observations at 9:15 a.m., S1 failed to have a BACK TO SLEEP sign posted in the rooms where infants sleep. A sign was posted prior to the Specialist departure. Corrective Action: Effective 6/23/2025, S1 stated she will immediately post a Back to Sleep sign in the infant classroom and ensure the sign is always posted to ensure compliance with this regulation.
