Based on observations and interviews on 03/31/2025 at 1:00 p.m., S9 (DOH 07/01/2022) failed to ensure children were supervised at all times during rest time while at the center. On 03/31/2025 at approximately 12:05 p.m., while children were napping, S9 failed to actively supervise children while asleep. During a walkthrough of the center, the Specialist observed S9 on her cellular phone while children were napping. S9 stated that she was on the phone with a doctor. She ended the call before the Specialist left the classroom and appropriate rest time…
