On January 13, 2026, a child (5 years of age) enrolled in Room 5 was left unsupervised after morning drop-off from 8:20 to 9:32 AM. A staff member found the child sitting in an adult restroom located in the hallway of the facility.

Concrete in the outdoor play area contains several gaps ranging from .5 to 2 inches, creating a tripping hazard for children. The northeast side of the playground has a gap between the ground and the bottom of the fencing that ranges from 2.5 to 4 inches.

Medication administration training is not on file for eleven current staff members.
