Based on record review 10:30 a.m., S1 failed to have documentation that 1 of 34 staff, S23 (FD: 11/19/2025) received orientation within seven days of the first day present at the center and prior to having sole responsibility for any children. S1 failed to have documentation that S23 completed DCFS Mandated Reporter Training. This was not corrected during the inspection. Corrective Action: Effective 01/20/2026, S1 stated staff will complete trainings prior to their first day at the center, to ensure compliance with this regulation.

Based on observation/record review at 10:50 a.m., S1 failed to ensure S12 has a current certification in pediatric first aid and CPR through training approved by the Department within 90 calendar days from the date of hire on 11/05/2025 and prior to assuming sole responsibility for 6, 1-year-old, children.
