Based on observation at approximately 11:00 a.m., S1 failed to the ensure the outdoor play space was enclosed with a permanent fence or other permanent barrier failed that protects children from traffic hazards, prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons. The exit gates on the playground closest to building number two failed to have a locking mechanism. This could not be corrected during the inspection. Based on observation/interview at approximately 11:15 a.m., S1 failed to ensure the center vehicle was maintained in good repair. The White Ford Transit Van would not crank. S1 stated the owners were aware of the maintenance requirement and planned to have the vehicle repaired. This could not be corrected during the inspection. Corrective Action: Effective 2/10/2026, S1 stated the vehicle will be picked up for repair by 2/13/2026, to ensure compliance with this regulation.
