Based on record review and interview at 1:00 p.m., the center's daily attendance record for children on 5/6/2026 did not accurately reflect the children on the child care premises at any given time, as 66 children were present and 59 children were signed in on the Brightwheel app. Corrective Action: Effective 5/6/2026, S1 stated she will review the daily attendance log daily around 9:00 a.m. for accuracy to ensure compliance with this regulation.

C.: Based on record review and interview at 1:00 p.m., S1 failed to provide documentation that at least two staff members are trained in medication administration every two years. Certification for S2 and S3 expired on 4/17/2026. S1 stated all staff will be taking the course on 5/13/2026. Corrective Action: Effective 5/6/2026, S1 stated staff will complete the class immediately and prior to its expiration date every two years to ensure compliance with this regulation.
