      The resilient material pea gravel located under the double arch climber located on the outdoor play area measured at a depth of under one (1) inch in area directly under the arch climber. Children were playing on the arch climber at the time of inspection.

    • All Personnel and Volunteers at the Center Must Demonstrate Knowledgeable Decision-making, Judgment, and Concern for the Proper Care and Well-being of Children.2.507.A

      Interviewed two (2) staff members that were present during the injury. Learned through staff member interviews that a ramen noodle cup was brought to staff member1 by a child in care and the staff member was asked to heat the noodle cup up in the mircrowave for the child. The staff member heated the noodle cup and water in the microwave and then removed the cup and set it on a lunch table near the microwave. The child picked up the noodle cup when the staff member was not paying attention to it and went and sat down at a lunch table. The hot noodles and water spilled when the child put a fork in the cup. The hot water and noodles caused a burn.
