Several classrooms and the kitchen have refrigerators with the interior and exterior has dried on food soil/debris. Dragonflies: The exterior door to the playground is lacking a door sweep; light can be seen under the door; not in good repair. There is a double baby stroller that has cloth seats and it is stored outside where it is wet and covered with leaves/debris.

Grasshoppers: Gloves stored on a shelf and an unlocked cabinet has toothpaste, sunscreen, Lysol spray and glass cleaner in the children's bathroom. Dry erase board cleaner and magic erasers on a shelf. Walking Sticks: Essential oils on a shelf. Wall paint and cleaners in an unlocked cabinet under the diaper changing table. Caterpillars: Hand sanitizer, adult scissors on a low shelf.

Dragonflies and Grasshoppers: Toilets are taped off indicating not in use; not in good repair.

Several classrooms have outdated lesson plans or are not following the written lesson plan that is posted. When staff were asked about the daily learning experiences, the staff did not have any materials prepped.

Children's coats and bags are co-mingled on the hooks in several classrooms.

A child in care under the age of 2 years old was given an over the counter medication (pain reliever) and the dosage instructions were not followed; a doctor's note was to be provided prior to administering. Several children under 2 years of age have over the counter medications and they cannot be administered without a doctor's note which is not on file. Another child in care was given "Smoothlax" (laxative) and the dosage instructions indicate that it cannot be given to a child under 16 years old without a doctor's note; a doctor's note is not on file.

Several children in care have prescription inhalers that are not in the original pharmacy container.

Several classrooms have blankets that children use co-mingled; not stored in a sanitary manner.

There is a broken wooden shopping cart and a trike that is missing a wheel; not in good repair. Toddler playground: There is a rake in the playground area accessible to children.

There is no clearly visible thermometers stored in the refrigerators in the Kitchen and Caterpillar room.

Grasshoppers: Toothbrushes are not stored in a sanitary manner; the bristle end of the toothbrushes are touching each other.
