      Hazardous items as follows were accessible to children: an air freshener spray. The item(s) was/were located in a staff bathroom. The door to the bathroom was wide open , the door from preschool classroom was also open, presenting possible hazardous situation.

      Children were not restricted to approved space as evidenced by the door to the staff bathroom, kitchen and office , as well as the door to a furnace storage were no closed/locked .
