290-2-3-.19(1)(a)1 requires that cribs and other equipment approved for infant sleep shall be in good repair and free of hazards. Stack cribs and cribs with drop sides shall not be used. It was determined based on observation that the black pack-n-play had a rip that measured five inches long where the mesh meets the bottom that posed a potential hazard.
