      1713.A.&B.C: Based on record review and interviews at 10:30 a.m., S6, failed to ensure all children were being supervised at all times. On 3/30/2026, at 10:18 a.m., C1, 18-months-old, was found alone on the center’s hallway by S8. C1 exited Classroom 3 at 10:17 a.m., and was unsupervised for one minute. This could not be corrected prior to the Specialist departure. S6 was suspended with pay for two weeks beginning 3/31/2026-4/12/2026. On 4/13/2026, S6 was suspended without pay until 4/21/2026.       C.: Based on record review/interview at 1:00 a.m., S1 failed to have documentation S17 completed LDE Key Training Module 1 within 7 days of the first day present at the center: -S17's hire date and first day present at the center was 9/9/2025. Module should have been completed by 9/16/2025 and Modules 2 and 3 by 10/9/2025. This was not corrected prior to Specialist departure.
