      Based on observation/record review at approximately 11:40 a.m., S3 failed to meet the required child to staff ratio for children of the following ages when she walked her class from the cafeteria back to the classroom: 15 children ages 3 and 4-years-old were with 1 staff. S2 was left in the cafeteria with 1 child. The required mixed ratio for children of this age is 14 children per 1 staff person. An additional staff person was needed.       1919.B.Based on observation at 11:35 a.m., menu substitutions or additions failed to be posted, written or electronically, on or near the menus as the children had chicken patties on a bun, with french fries, fruit, and milk but the menu identified that they should have eaten chicken noodle soup, mixed vegetables, diced pairs, and saltine crackers. S8 added the substitution prior the Specialist leaving the center. Based on record review at approximately 1:45 p.m., the current passenger transportation log provided by driver or attendant was inaccurate as the time children (C1 C2) were placed on the vehicle for 5/1/2025 was prefilled.
