Based on observation/record review at approximately 11:40 a.m., S3 failed to meet the required child to staff ratio for children of the following ages when she walked her class from the cafeteria back to the classroom: 15 children ages 3 and 4-years-old were with 1 staff. S2 was left in the cafeteria with 1 child. The required mixed ratio for children of this age is 14 children per 1 staff person. An additional staff person was needed.
