Based on record review/interview at 10:28 a.m., S1 failed to meet the required child to staff ratio for children of the following ages: 13 children ages, 2-to-4-years-old, with 1 staff, S10. The required ratio for children of this age is 12 children per 1 staff person. S2 corrected and moved a child into S11's classroom. Corrective Action: Effective 7/7/2025, S1 stated she will pay closer attention to child to staff ratio during morning drop off and adjust accordingly, to ensure compliance with this regulation.

Based on record review at 10:15 a.m., S1 failed to have an application/staff information form that included S20's last date of employment and reason (termination or resignation) for leaving employment. This was corrected during the inspection. Corrective Action: Effective 7/7/2025, S1 stated she will conduct a review of all staff records prior to moving from a filing of active to inactive, to ensure compliance with this regulation.

This was corrected during the inspection. Based on observation/record review at 9:15 a.m., the center's staff and owner's daily attendance record failed to accurately reflect persons on the child care premises at any given time. S1 was present and not signed in on the record. S1 also failed to sign out on the record on 10/25/2024. S1 corrected this during the inspection.
