A 4 month old infant placed in an unsafe sleep environment. Observed said infant was swaddled while sleeping in a pack n play in the main play room on the main floor of the home. Observed no evidence of a health care plan completed and signed by the child's physician stating child is allowed to be swaddled while sleeping.
