The facility did not immediately report a critical incident to the parent of a child (three years and nine months of age) who was left unsupervised in Europe Room 9 on August 18, 2025.

On August 18, 2025, a child (three years and nine months of age) was left unsupervised in Europe Room 9 for approximately one to two minutes, while the rest of their class transitioned to another unit.
