Based on record review at 12:30 p.m., S1 failed to request and obtain from the Department a new CCCBC-based determination of eligibility for child care purposes for S3 at least once during a five-year period. S3's CCCBC expired on 6/19/2025 and worked in the center on 6/20/2025, 6/23/2025, 6/24/2025, and 6/25/2025. S3 received a CCCBC-based determination of eligibility on 6/26/2025. Corrective Action: Effective 8/26/2025, S1 stated that she will check the CCCBC messages weekly, to ensure compliance with this regulation.
