The requirements for protective outlet covers or twist-lock outlets were not met as evidenced by electrical outlets were not covered in that the school age classroom in the back of the building.

The ceilings located in the purple classroom and the yellow classroom was/were not in good condition as evidenced by there were 13 ceiling tiles in the purple room with water damage which resulted in brown spots and 6 ceiling tiles with the same damage in the yellow classroom.
