      Protective covers were missing from the smaller rooms in the back of home, where they change diapers, and an additional play area.

      There were 2 employees with expired TB test.

      There were things that say keep out of reach of children within reach of the children. There was spray in the bathroom, and the laundry room in the basement needs to stay locked there was storage that had multiple things accessible in that room.
