Based on record review and interviews at 3:05 p.m., S1 failed to notify LDOE and Child Welfare within 24 hours of the following critical incident : On 1/8/2026, at 10:00 a.m., O1, C1's Mother, informed S1 that C1, age 2-years-old, was pinched and poked by S5 while in the classroom on an unknown date and time. Specialist was unable to validate the allegations. Corrective Action: Effective 1/15/2026, S1 stated she will report all reportable incidents to the Department within 24 hours of knowledge of the incident to ensure compliance with this regulation.
