Per staff collaterals and staff admission, a 2-year-old-child's diaper was changed outdoors on the playground, on a blanket. The facility did not ensure diapering was conducted on an approved diaper-changing surface.

Per staff collaterals, in Room 103 a staff member was observed on 2/13/2026 dragging a 3-year-old-child by their hoodie, twisting the hood while the child's face turned red.

Staff did not practice methods of behavior that are designed to help children develop inner controls and manage one's own behavior when a staff member from Room 103 admitted to physically holding a 3-year-old-child around the waist and forced them to remain seated in time-out for the full duration. Per staff collaterals, a 2-year-old-child was placed alone in the bathroom in Room 103 in an attempt to redirect their behavior.

The facility did not implement behavior management practices as stated in the facility's written discipline policy when the discipline policy does not permit staff to restrain a child for disciplinary purposes.

Per collateral contacts, children are not properly cleaned during diaper changes.

Diaper procedures were not followed in Unit E 119 when the staff did not remove the soiled gloves before placing on the clean diaper.

Out of 12 children's files reviewed, two children do not have documentation of current immunizations on file.

In room E, there were open goldfish crackers, club crackers and cheese crackers, that were not stored in airtight containers.

Per staff statement, two infants, one 11 month old and one 12 month old who is not walking, nap on a cot and not in a crib or playpen.
