1507.A.1. Based on record review at 11:24 a.m., the daily attendance log for children failed to include the first and last name of the person to whom the child was released. The Specialist reviewed attendance records from 11/3/2025, until present, and observed the first and last name of the person to whom the child was released failed to be documented 13 times. Corrective Action: Effective 11/13/2025, S1 will review the children's attendance record prior to the end of the day to ensure compliance with this regulation.

Based on record review at 11 a.m., the visitor daily attendance record failed to include departure times and the purpose of the visit. The Specialist reviewed the visitor log from 8/20/2025, until present, and observed the departure time failed to be documented 2 times and the purpose of the visit failed to be documented 2 times. Corrective Action: Effective 11/13/2025, S1 will review the visitor's attendance record prior to the end of the day to ensure compliance with this regulation.

Based on record review at 12:55 p.m., the daily attendance log for children failed to include the first and last name of the person to whom the child was released. The Specialist reviewed the daily attendance record from 7/25/2025 through 7/31/2025, and verified the first and last name of the person to whom the child was released failed to be documented 16 times. Corrective Action: Effective 8/13/2025, S1 will review with all staff and parents that the first and last name of the person to whom the child is released must be included to ensure compliance with this regulation.

Based on record review at 12:50 p.m., the visitor daily attendance record failed to accurately reflect when a visitor was on the child care premises. The Specialist reviewed the visitor log from 4/2/2025, until present, and observed the departure time failed to be documented 3 times. Corrective Action: Effective 8/13/2025, S1 will remind staff that a departure time for all visitors must be included on the visitor log and she will review the visitor log at the end of each day to ensure compliance with this regulation.

Based on observation at 9:20 a.m., strings and cords failed to be inaccessible to children under age 4. The Specialist observed a microphone cord, an extension cord, and a refrigerator cord accessible to 8 children, ages 2-years-old through 3-years-old, in S1's classroom. This was corrected prior to the Specialist exiting the center. Corrective Action: Effective 8/13/2025, S1 will have closing staff complete a walk-through of the center at the end of each day to look for visible cords to ensure compliance with this regulation.

Based on record review at 11 a.m., S2 failed to post 'The Safety Box' newsletter issued by the Louisiana Office of the Attorney General. The most recent edition of 'The Safety Box' posted was dated January - March 2023. This was corrected prior to the Specialist exiting the center. Corrective Action: Effective 8/13/2025, S1 will print, review, and post all new editions of 'The Safety Box' quarterly to ensure compliance with this regulation.

Based on observation at 9:40 a.m., the outdoor area failed to be free of hazards. The Specialist observed wire, approximately 6 feet in length and connected to the fence, protruding from the bottom of the fence. This was unable to be corrected prior to the Specialist exiting the center. Based on observations, S1 and S2 failed to prevent children under age 4 from eating foods that are implicated in choking incidents. At 10:30 a.m., the Specialist observed 3 children ages 1-year-old, eating Veggie chips in S2's classroom. At 11:45 a.m., the Specialist observed 8 children, ages 2-years-old through 3-years-old, eating chicken nuggets, which failed to be cut in pieces smaller than can be swallowed whole, in S1's classroom.
