      1713.A&B&C.: Based on observations at 1:44 p.m., S2 failed to supervise children at all times. The Specialist observed C1, 10-years-old alone in a classroom, awake and lying on the floor watching a tablet during naptime. The Specialist did not observe any staff in the classroom to supervise the child. S2 went into the classroom at 2 p.m. Corrective Action: Effective 5/21/2026, O1 stated she would make sure all children are in the classroom asleep in their appointed areas, ensuring compliance with this regulation.

      Based on observations at 1:45 p.m., S1 failed to keep strings and cords, including but not limited to those found on equipment, window coverings, televisions, and radios, inaccessible to children under age 4. The Specialist observed an exposed cable/electrical cord on the playground and accessible to children.
