Per video footage from 04/14/2025, a staff member used forms of corporal punishment while managing a four-year-old child's behavior. Staff member pulled and pushed the child, attempting to keep the child seated in one spot. Staff member used their hand to smack the side of the child's head, on the cheek. Child attempted to hold their head after being hit. Staff member grabbed the child by the wrist, while the child asked "Why'd you hit me? You hit me." Staff member proceeded to yank the child upwards by the arms, forcing the child to stand up and leave the classroom together.

Emergency plans are not developed to include a serious injury and lack detail specific to the location of the facility.

Specialist observed diapers disposed of in an uncovered container in Room 123.

Hot water in Room 112, 123, and the multi purpose room exceeded 110 degrees. Water temperature ranged from 121 to 123 degrees.

Dawn dish soap, hand sanitizer, Clorox wipes, lice shampoo and other items bearing warning labels located in the multipurpose room are not in locked storage.
