      1507.A Based on interviews and record review at approximately 4:00 p.m., S4 failed to ensure the daily attendance record for children accurately reflected the children on the child care premises at any given time. On 4/14/2026, C1 was present at the center but he was not signed in or out. This could not be corrected during the inspection. Corrective Action: Effective 5/15/2026, S4 stated she will make the opening and closing persons responsible for checking the attendance log for accuracy twice daily to ensure compliance with this regulation.

      Based on interviews at approximately 4:00 p.m., S3 failed to ensure that a child too large to be restrained using the manufacturer’s restraint device shall not be placed in equipment. S3 stated that on date unknown, C1, 1-year-old, who was too large, was placed in an exersaucer which caused him to have indentions in his back. S3 stated C1 was never put back in the device after being informed of the indentions.       Based on observations at approximately 10:30 a.m., S3 failed to have written authorization from a physician for C3, 4-months-old, as required for infant to sleep in a bouncer. The Specialist observed C3 asleep in a bouncer while completing the walk through of the center. S3 removed C3 from the device and placed her in her crib prior to the Specialist leaving classroom. Corrective Action: Effective 5/15/2026, S4 stated she will re-train all infant staff on the importance of transitioning infants to their cribs once asleep by 5/22/2026, to ensure compliance with this regulation.

The Specialist observed C2 was asleep on the floor during the walk through of the center. S3 removed the bib and placed C2 in her crib prior to Specialist leaving the classroom. S11 stated there will be a staff delegated to review the outdoor area daily for hazards to ensure compliance with this regulation.

      Based on observations at 9:45 a.m., S10 failed to ensure that the manufacturer's restraint device was fastened as Specialist observed C3, 2-months-old, asleep and unrestrained in a bouncer. Corrective Action: Effective 11/20/2025, S1 stated she will reiterate with all staff that the manufacturer's restraint must be fastened at all times when occupied by a child to ensure compliance with this regulation.

Corrective Action: Effective 11/20/2025, S1 stated she will assign the daily morning floater as the person responsible for checking that the daily observations are complete to ensure compliance with this regulation.

      1919.H. Based on interviews at 9:45 a.m., infants failed to be held while bottle fed. S16 was observed on several occasions (dates unknown) having propped up C7, 9-months-old, bottle. This could not be corrected.       Based on observation at approximately 11:45 a.m., the Specialist observed while completing a walk through of the center a pacifier was attached to a child (C5) while he was sitting in a bouncer in S3's classroom. S3 removed this pacifier from C5's gray and blue bib prior to the Specialist leaving the room. Corrective Action: Effective 8/29/2025, S1 stated she will retrain staff on the importance of ensuring that pacifiers are not attached to the clothing of a child by 9/5/2025 to ensure compliance with this regulation.
