C.: Based on record review/interview on 6/12/2025, at 4:00 p.m., S1 failed to provide documentation that 1 of 5 staff, received orientation within seven days of the first day present at the center and prior to having sole responsibility for any children. S4 was observed supervising six children, aged 1-2 years-old in the classroom alone at the time of the inspection. S1 stated S4 has completed the orientation training, but it was not available at the center for verification.
