Based on observation on 5/5/2026, at 12:15 p.m., although the center has a Behavior Management Policy implemented, S6 (DOH 2/20/2025), used a prohibited method of discipline as C2, age 3-year-old, was subject to verbal discipline. The Specialists observed S6 aggressively yelling at C2, telling her to lay down on your cot and don’t get up during nap time. S1 issued a written reprimand to S6 addressing the behavior management policy. Corrective Action:

Based on record review on 5/5/2026, at 12:00 p.m., S1 failed to have documentation that all staff on the premises and accessible to the children have current certification in pediatric first aid through training approved by the department. S5 did not have documentation of this certification. Corrective Action:

C. Based on record review on 5/5/2026, at 12:00 p.m., S1 failed to have at least two staff members trained in medication administration whether the early learning center administers medication or not. No staff had current certification. Corrective Action:

1915.A. Based on record review on 5/5/2026, at 1:45 p.m., S8 failed to have documentation of an explanation when something was observed on C2, 3-years-old, upon arrival to the center on the following days: 4/28/2026 and 4/29/2026. Corrective Action:

1723.A.&B.: Based on record review at 2:00 p.m., S1 failed to provide documentation that all staff on the premises and accessible to the children have current certification in infant, child, and adult CPR through training approved by the Department. S6's certification expired on 8/31/2025. Corrective Action: Effective 11/6/2025, S2 stated she will set a reminder 1 month prior to expiration for staff to renew their infant, child, and adult CPR certifications to ensure compliance with this regulation.

S6's certification expired on 8/31/2025. Corrective Action: Effective 11/6/2025, S2 stated she will set a reminder 1 month prior to expiration for staff to renew their Pediatric First Aid certifications to ensure compliance with this regulation.

1719.C.Based on record review at 11:30 a.m., S2 failed to have documentation that S11 (DOH 2/21/2025) completed the LDE Key Orientation Training Modules 2 and 3 within thirty days present at the center. Corrective Action: Effective 6/30/2025, S2 stated she will set a reminder for required training within the first 20 days for all news hire to ensure compliance with this regulation.

1723.F. Based on record review at 11:30 a.m., S8 (DOH 6/10/2025) failed to have current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. The Specialist observed S8, alone, supervising 6 children, ages 2 - years-old. This was corrected when S8 combined the children with S3. Corrective Action: Effective 6/30/2025, S2 stated she will have all staff receive required training prior to assuming sole responsibility for any children to ensure compliance with this regulation.

1727.A.B.Based on record review at 11:30 a.m., S2 failed to have documentation that S7 completed the annual online child abuse and neglect Mandated Reporter Training provided by DCFS. S7's certificate expired on 3/15/2024. Corrective Action: Effective 6/30/2025, S2 stated she will set a reminder 3 days prior to expiration date to ensure compliance with this regulation.

1807.B.Based on record review at 10:45 a.m., S2 failed to have documentation of S8's (DOH 6/10/2025) said determination available on the centers CCCBC roster at all times. Corrective Action: Effective 6/30/2025, S2 stated she will place all staff with an eligible CCCBC on the centers CCCBC roster to ensure compliance with this regulation

1719.B. Based on record review at 1:30 p.m., S2 failed to have documentation that S12 (DOH 11/8/2024) completed the DCFS online Mandated Reporter Training within seven days of the first day present at the center.

Based on record review at 1:30 p.m., S2 failed to have documentation that 1 of 10 staff on the premises and accessible to children have current certification in infant, child, and adult CPR through training approved by the Department. S5 (DOH 7/3/2024) failed to have the current certification.

1723.C. Based on record review at 1:30 p.m., S2 failed to have documentation that 1 of 10 staff on the premises and accessible to children have current certification in Pediatric First Aid through training approved by the Department. S5 (DOH 7/3/2024) failed to have the current certification.

C. Based on record review at 11:00 a.m., S2 failed to immediately notify the Parent, of the following critical incident: On 10/22/2024 at 10:59 a.m., S11 slapped C1, age 3-years-old, in the mouth for using foul language. O1 was notified on 10/22/2024 at 1:09 p.m. LDOE and DCFS was notified on 10/22/2024.

Based on interviews and record review (video footage) at 11:15 a.m., although the center had a written behavior management policy implemented, S11 (DOH 6/18/24) used a prohibited method of discipline as C1, age three-years-old, was subject to physical discipline. On 10/22/2024 at 10:59 a.m., S11 slapped C1 in the mouth for using foul language. S11 was terminated on 10/22/2024.

1715.A.1. Based on record review at 11:15 a.m., S2 failed to have an application/staff information form to include the name, date of birth, home address, and phone number, training, work experience, and educational background for S11 (DOH 6/18/2024).

1717.A. Based on record review at 11: 30 a.m., S2 failed to have documentation on file that included person's name, address, phone number, list of duties performed while at the center for O2.

C. Based on record review at 10:45 a.m., S2 failed to have documentation that S7 (DOH 8/15/24) and S9 (DOH 10/22/24) completed the DCFS online Mandated Reporter Training within seven days of the first day present at the center and prior to having sole responsibility for any children.

1915.A. Based on record review at 11:35 a.m., S2 failed to document observations, when something is observed, noted on children upon arrival to the center on the following dates: 7/22/2024 - 7/26/2024, 8/22/2024, and 8/23/2024.
