      1725.C. Based on record review at 11 a.m., S1 failed to ensure at least one staff member on the premises was trained in medication administration. S2's medication administration training expired on 02/20/2026, and S9's medication administration training expired on 2/22/2026. Corrective Action: Effective 04/10/2026, S2 stated she will set a reminder 30 days prior to expiration to ensure compliance with the regulation.

      1807.B. Based on record review at 10:30 a.m., S1 failed to have documentation of eligible staff CCCBC determination available for S9 on the center’s CCCBC roster at all times. This was corrected prior the Specialist’s departure. Corrective Action: Effective 04/10/2026, S1 stated she will add all new employees to the CCCBC roster and check it monthly to ensure compliance with the regulation.

Based on interviews and record review on 1/22/2026, at 12:30 p.m., S1 failed to notify the parent immediately of the following critical incident: On 1/20/2026, S7 left C1, 2-years-old, alone inside classroom 3 unsupervised at 3:32 p.m. C1 was discovered by S1 at 3:46 p.m. C1 was unsupervised for 14 minutes. The parent was notified on 1/20/2026, at 4:30 p.m., by S1 at pick up. Corrective Action: Effective 2/11/2026, S2 stated that all parents will be notified immediately of all incidents and accidents to ensure compliance with this regulation.

On 1/20/2026, S7 left C1, 2-years-old, alone inside classroom 3 unsupervised at 3:32 p.m. C1 was discovered by S1 at 3:46 p.m. C1 was unsupervised for 14 minutes. S7 was terminated on 1/21/2026. Corrective Action: Effective 2/11/2026, S2 stated that all staff will document of tranistion logs and complete a name-to-face count when leaving an area of the center to ensure compliance with this regulation.
