On 3/30/26, upon Licensing Specialist arrival to the facility, a 9-year-old, enrolled child approached the Licensing Specialists vehicle, in the parking lot. No staff members were present with the child at the time.

In Room 1, there are ten children and one teacher present on the playground. The classroom is over ratio by four children. In Room 2, there are 17 children and one teacher present in the classroom. The classroom is over ratio by five children.

Of the 10 staff files reviewed, one does not have health and safety training within the required timeframe.

Of the 10 staff files reviewed, one has a record of an expired certification (12/2025).

Of the 10 staff files reviewed, three do not have a record of orientation.

Of the 10 staff files reviewed, five do not have annual hours for the 3.1.25 to 2.28.26 licensing year completed.

Of the 10 staff files reviewed, one does not have a record of a negative tb test and one has an expired health status form.

There is not a record of annual practices including the date and time of each practice and a list of all participating staff members.

In Room 2, there is a school-aged child (9 years old), enrolled. The room is licensed for preschool only.

In Room 1, there are disinfecting wipes on a shelf out of locked storage. Corrected at visit. There is a large box of diaper wipes on the ledge in between classrooms 1 and 2, less than five feet above the floor. In the Room 1, the baskets holding diaper wipes is four feet, eight inches above the floor. In Room 2, the baskets holding diaper wipes are four feet, ten inches above the floor.

There is a deep gash in the wall by the Room 2 cots that has exposed drywall.

There is a power strip that has an uncovered outlet.

There is not a signed parental permission form to review for an enrolled school-aged child to walk to and from activities away from the facility.

Program Director did not act with sound judgment when an unenrolled, ill, school age child was allowed to be present in Room 2 (South).

There are many areas of the playground where pieces of landscaping tarp are exposed and buckled. There are three metal pieces with sharp edges on the fence. There is one wooden fence slat that is broken with splintered edges.
